The following information is an overview of the Point of Service (POS) plan. Please use the chart and points listed below to better understand how the plan works.

• You choose a Primary Care Physician from the OSF HealthPlans Provider Directory to coordinate your healthcare.
• With the Point of Service plan, you have two options when you are in need of medical care.

OPTION 1: This option works like the OSF HealthPlans HMO and is the least expensive of the two options.

OPTION 2: This option works like a traditional health insurance plan. Although your cost is higher than with the other option, you have more flexibility. With this option you are able to choose any doctor outside the OSF HealthPlans network or refer yourself to an OSF HealthPlans specialist without first seeing your Primary Care Physician.
