integrating our organizations to provide you with enhanced benefits
and options. Both OSF HealthPlans and Humana are committed to
maintaining a positive relationship that focuses on high quality
customer service and commitment to guidance. For questions specific
to your plan, please see the customer service number on the back of
your ID card. Your member identification card is the key to guiding
you to the customer service options specific to you and your plan.
Members may request
individually identifiable health information to be disclosed to another
party. However, OSF HealthPlans must have written authorization to
release this information. This disclosure form should be completed and
sent to OSF HealthPlans for us to release the information.
